Thyroglobulin Antibody (TgAb)

Thyroglobulin antibody (TgAb) testing can be a tool for assessing thyroid health during pregnancy. Elevated levels may indicate autoimmune conditions, like Hashimoto’s thyroiditis, that are associated with an increased risk of pregnancy complications. This article covers TgAb reference ranges, research on elevated levels, and common causes to support better maternal and fetal outcomes.

TgAb Testing

The thyroglobulin antibody (TGAb) test measures the presence of antibodies against thyroglobulin. 

Thyroglobulin is a large glycoprotein complex. It is the storage form of thyroid hormone within the colloid space of thyroid follicles. Thyroglobulin antibodies are those that target thyroglobulin. These antibodies can be present in both hypothyroidism and hyperthyroidism.

TgAb in Pregnancy

It is not uncommon for thyroglobulin antibodies to increase in the first trimester if present preconception. TgAb levels tend to mimic the increase and decrease seen with TPOAb, with a potential rise in the early first trimester and a subsequent decrease throughout the remainder of pregnancy.
Elevated thyroid antibodies are found in between 60% and 85% of women presenting with postpartum thyroiditis.

High TgAb in Pregnancy

​Most studies have focused on the presence of thyroid peroxidase antibodies (TPOAb) and 
less on the presence of TgAb; therefore, there is limited research on the effects of these antibodies on pregnancy outcomes. A 2023 systemic review and meta-analysis did find a correlation between TgAb and recurrent pregnancy loss.

Most Common Causes of High TgAb in Pregnancy

  • Environmental Chemical Exposure 
  • Heavy Metal Exposure
  • Mycotoxin Exposure
  • Iodine Overdose
  • Latent Viral Infections
  • Lyme Disease
  • Dysbiosis
  • Gastrointestinal Pathogens (Parasites, Bacteria, etc...)
  • Chronic Infections
  • High Dose Biotin Supplements
  • Certain Medications (Accutane, Interferon) 
  • Nutritional Deficiencies (vitamin D, magnesium, selenium)
  • Genetics

Low TgAb in Pregnancy

Not considered pathogenic
